2025 Annual Celebration Award Nominations
Nominations will be accepted for both those we serve and the providers/community partners who support them. Your Nominations are critical in helping us acknowledge and celebrate the hard work and commitment of those we serve and our providers.
We encourage you to submit multiple nominations. You may nominate the same person or organization for different awards.
Nominations will be accepted until March 7, 2025.
For individuals receiving LifeWays services or services through LifeWays' provider network.
For provider agencies, community organizations, or individual service providers. (Excluding LifeWays staff)

Award Categories
Consumer Awards:
Courage to Fight Back Award: This award is given to an individual who has demonstrated extraordinary courage in breaking barriers, overcoming obstacles, supporting others, and motivating others.
Rising Star Award: This award is presented to an individual who has risen above the challenges they face and has demonstrated they are on their way to the life they desire.
Shining Star Award: This award is given to an individual who has inspired those around them to reach for the stars, despite the challenges they may face.
Spirit of Hope Award: This award honors an individual who has kept the spirit of hope alive, regardless of the obstacles that may stand in the way.
Determination Award: This award is presented to an individual who has continued to take steps toward their goals despite challenges and setbacks.
Pathfinder Award: This award recognizes an individual who has gone ahead in their recovery, discovered a new way, shown others the path.
Provider/CommunityAwards:
Compassion Award: This award honors an individual or organization who has supported someone’s journey to recovery with sensitivity and compassion.
Innovation Award: This award is presented to an individual or organization that has improved opportunities by embracing a culture of continuous improvement and finding new and progressive ways to meet challenges.
Exceptional Service Award: This award is given to an individual or organization that has exemplified a commitment to ensuring all customer interactions exceed expectations.
Integrity Award: This award is presented to an individual or organization that has demonstrated commitment to the values we uphold through personal accountability and adherence to professional ethics.
Person Centered Care Award: This award recognizes an individual or organization that empowers the people we serve to achieve their hopes, dreams, desires, and goals through treatment focused on the individual’s needs and preferences.
Strategic Partnership Award: This award is given to an individual or organization that has created a successful partnership to improve the well-being of those we serve and the community.
