2021 Person-Centered Care Award

Criteria: An individual who directly or indirectly contributes to improving the lives of consumers. This person continually seeks opportunities to support recovery and encourage consumers to realize their true potential. This person maintains a focus on service(s) that will have a positive impact on, and improve the quality of, life for our consumers. Any staff member qualifies for this award. Anyone may submit nominations for any staff member.
