2019 Annual Celebration Ticket & Sponsorship Form

It’s time for LifeWays Community Mental Health to recognize the many achievements of the consumers we serve and our network of providers at our Annual Celebration on Thursday, May 16, 2019. Event sponsorships available include a reserved table and an ad in the program.

2019 Sponsorship Levels

  • Champion Level $200 ($225 after April 5, 2019) A full-page ad and one reserved table (8 seats)

  • Advocate Level $150 ($175 after April 5, 2019) A ½-page ad and one reserved table (8 seats)

  • Supporter Level $125 ($150 after April 5, 2019) A ¼-page ad and one reserved table (8 seats)

Advertising space in the program cannot be guaranteed for any ad received after April 30, 2019.

Tickets to the event are required by all guests for admittance. Free general admission tickets are available on a first-come, first-served basis for individuals who currently receive services through LifeWays Community Mental Health and our Provider Network only.

To purchase tickets to this year’s event, please return this form with payment attached no later than April 30, 2019. Tickets will be mailed within 5-10 days or will be available for pick up. General admission tickets are $10 each ($15 after April 30, 2019) while tickets last.
